El AUSENTE “OJO CLINICO”

La  ciencia y arte de la medicina  en su evolución se ha trasformado constantemente. La medicina que se practicaba en el siglo pasado, principalmente entre los años de 1940 a principio de 1960, estaba muy relacionada a las enseñanzas del viejo continente y específicamente a los grandes clínicos franceses Sin embargo hemos perdido mucho de lo que en un excelente artículo, Reminiscencias del “ojo clínico” (Francisco González Crussí,2019)*  menciona y cito:

El hecho de haber estudiado medicina tuvo que ver con la influencia de un estudiante de medicina que en aquella época impartía la clase de bioquímica en el CUM (Centro Universitario México). Esa pasión y conocimiento, despertó en mi la intriga y deseo de conocer más sobre el funcionamiento y estructura del cuerpo humano.

La medicina en la época en que inicie la carrera, a mediado de los año ’70, aunque con gran influencia de nuestros vecinos del norte, aún tenia ese precepto del diagnóstico que González Crussí hace referencia.

La importancia de una buena semiología y exploración  ( de los autores Antonio y Juan Surós Batlló), debería ser vigente.

Yo la aprendía  en el salón de clases, con el profesor   y  el paciente presente en clase, el interrogatorio, palpacióin auscultación los diversos sonidos provocados por las lesiones de la tuberculosis. (Dr. Fernando Rébora Gutiérrez)

Posteriormente aplicaría esos conocimientos en la residencia en el Instituto Nacional de Ciencias Médicas y Nutrición “Salvador Zubirán (INCMNSZ o INNSZ) y en las clases de Bioquímica, Anatomía, Fisiología y Doble Titulación, que impartí en la Universidad (La Salle Bajío) durante muchos años.

El ojo clínico era desarrollado con base al estudio y la adecuada exploración e interrogatorio durante la entrevista con el paciente. Los médicos hemos perdido ese “ojo clínico” porque hemos sustituido la paciencia de un buen interrogatorio y exploración por la tecnología y múltiples pruebas de laboratorio, con la necesidad de ver más y más pacientes, no solo en forma particular, también la saturación en las Instituciones gubernamentales de salud.

La medicina en nuestro país se ha transformado en un proceso industrial de ensamblaje que favorece la producción en masa de estudiantes con licencia en medicina.

Todo esto tiene sus raíces en un movimiento, que este noviembre cumple 55 años.

En esa época, se iniciaron una serie de huelgas para mejorar las condiciones tanto de salario como prestaciones de internos y residentes.

Esta “inocencia” política fue, probablemente, uno de los factores que más influyeron en la desaparición fugaz del movimiento. Las demandas hechas por los médicos internos y residentes, en 1964, fueron tan legítimas como justas; no pretendían elevarse como casta superior, ni tenían intenciones ajenas a la protección económica y laboral, mucho menos tenían “tintes comunistas”, como diría entonces el diputado Everardo Gámiz Fernández. (Cabello-López A 2015).

Hoy como en antaño siguen siendo vigentes las necesidades de los médicos a recibir una adecuada remuneración por sus labor, así como prestaciones, capacitación y reconocimiento, necesarias para su desempeño laboral.

El presidente López Mateos aseguró, en la creación del IMSS: “…el régimen de la seguridad social constituye uno de los propósitos esenciales de nuestras instituciones democráticas, uno de los mejores instrumentos para la realización de los postulados de justicia social”.

Sin embargo, continúa la represión, además de hostigamiento hacia los médicos y enfermeras, por la limitación de los postulados de la justicia social que han sido abanderados por muchísimos años.

Persiste la idea que los profesionales de la salud, principalmente los médicos, tenemos percepciones dentro del IMSS, ISSSTE, SSA, o en la práctica privada altísimas.

-Hoy, por eso, los comentarios de que apoyen a cubrir los medicamentos.

La realidad no es diferente que hace 50 años.

El multi-empleo, sigue siendo una práctica entre galenos y enfermeras. Sus percepciones  no permiten cubrir sus necesidades económicas, lo que actualmente provoca el síndrome del “burnout” (agotamiento, despersonalización, cinismo, desmotivación e insatisfacción en el trabajo, que conllevan a un pobre desempeño laboral) y como consecuencias las demandas por negligencia médica.

Un movimiento en 1964 generó la masificación de las universidades y la despersonalización de los médicos. El desprecio por la practica médica de gobiernos, sociedad, se generalizó.

El resultado lo vemos hoy, con las críticas acérrimas y el evidente ausente ojo clínico.

Las consecuencias, mi estimado lector son evidentes lo mismo que el mal necesario de la educación….
